Job Overview:We are seeking a dedicated and compassionate Band 6 Physiotherapist to join the team at Jean Hayes Reablement Unit. This role focuses on elderly patients requiring rehabilitation and community-based physiotherapy. The successful candidate will play a crucial role in enabling patients to regain independence and improve their quality of life.

Key Responsibilities:

  • Provide physiotherapy assessments and treatments for elderly patients, focusing on rehabilitation and mobility.
  • Work collaboratively with a multidisciplinary team to create individualized care plans.
  • Conduct home visits and community-based interventions as needed.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Support the discharge planning process to ensure safe and effective patient transitions.

Requirements:
