Job Overview:We are seeking a dedicated and compassionate Band 6 Physiotherapist to join the team at Jean Hayes Reablement Unit. This role focuses on elderly patients requiring rehabilitation and community-based physiotherapy. The successful candidate will play a crucial role in enabling patients to regain independence and improve their quality of life.
Key Responsibilities:
- Provide physiotherapy assessments and treatments for elderly patients, focusing on rehabilitation and mobility.
- Work collaboratively with a multidisciplinary team to create individualized care plans.
- Conduct home visits and community-based interventions as needed.
- Monitor patient progress and adjust treatment plans accordingly.
- Support the discharge planning process to ensure safe and effective patient transitions.
- Provide education and support to patients, families, and carers on physiotherapy techniques and goals.
Requirements:
- HCPC registration as a qualified Physiotherapist.
- Previous experience working with elderly patients, rehabilitation, or community-based care.
- Strong communication and interpersonal skills for effective teamwork and patient interaction.
- Ability to independently manage a varied and complex caseload.
- Commitment to patient-centered care and continuous professional development.
