Are you a compassionate and dedicated Social Worker looking to make a difference in the lives of adults with learning disabilities and autism? Join Surrey’s Adult Social Care Team, where you will play a vital role in supporting individuals to live independently, access the right care, and achieve their goals.

About the Role

As a Social Worker in the Learning Disability & Autism Team, you will work with adults with complex needs, ensuring they receive person-centred support and safeguarding. You will assess needs, develop care plans, and work with families, carers, and other professionals to provide the best possible support.

Key Responsibilities
  • Conduct strength-based assessments to identify the needs and aspirations of individuals with learning disabilities and autism.
  • Develop and review personalised care and support plans, promoting independence and well-being.
  • Work in partnership with health professionals, care providers, and voluntary organisations to ensure the best outcomes for service users.
  • Advocate for individuals, ensuring their voices are heard and they are fully involved in decisions about their care.
  • Undertake safeguarding assessments and interventions, ensuring individuals are protected from harm and exploitation.
  • Provide advice, guidance, and emotional support to families and carers.
  • Maintain accurate records and reports in line with statutory requirements and best practice.
About You

? A qualified Social Worker, registered with Social Work England.? Experience working with adults with learning disabilities and autism in a social care setting.? Knowledge of legislation, policies, and frameworks related to adult social care, including the Care Act 2014 and Mental Capacity Act.? Excellent assessment, communication, and problem-solving skills.? Ability to work collaboratively with multi-disciplinary teams and external agencies.? A commitment to promoting independence, dignity, and inclusion for people with learning disabilities and autism.
