Are you an experienced and dedicated Social Worker looking to take the next step in your career? Surrey’s Locality & Hospital Teams are seeking a Senior Social Worker to support adults with complex care needs, ensuring timely interventions, safe hospital discharges, and effective community support.
About the RoleAs a Senior Social Worker, you will play a key role in supporting vulnerable adults, older people, and those with physical disabilities or mental health needs. Working across locality and hospital settings, you will help individuals regain their independence, access appropriate care, and ensure smooth transitions between hospital and home.
Key Responsibilities- Manage complex caseloads, providing expert social work support across hospital and community settings.
- Undertake strength-based assessments and develop personalised care plans.
- Work in partnership with NHS teams, care providers, and community services to facilitate safe hospital discharges.
- Ensure timely interventions to prevent unnecessary hospital admissions.
- Provide mentorship and support to junior social workers and team members.
- Conduct Mental Capacity Act assessments and contribute to safeguarding investigations.
- Advocate for service users, ensuring they receive person-centred care tailored to their needs.
? A qualified Social Worker, registered with Social Work England.? Significant experience in adult social care, hospital discharge, or community-based support.? Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.? Excellent decision-making, risk assessment, and problem-solving skills.? Ability to work effectively with health and social care professionals to deliver integrated support.? A commitment to promoting independence, dignity, and choice for adults in need.
