The role:

Wrexham Children Services are looking for a Social Care Assessor to work alongside Social Workers in providing information, advice and assistance as the first point of contact for children in Wrexham.

The role will include undertaking 'What Matters" conversations with parents, carers and children, liaising with partner agencies across the statutory, public and voluntary sector, assessing care and support needs, and working with families to assist with improving their wellbeing.

The Social Services and Well Being (Wales) Act 2014 provides the legislative framework regarding the provision of information, advice and assistance and identifying needs for care and support of children. The Children's Single Point of Access are pivotal in ensuring children and families are supported to achieve their wellbeing outcomes through outcome focused conversations, providing information, advice and assistance; or identifying whether further assessments of care and support needs are required.
