The Patient Care Manager -Hospice assists Executive Administrator/Executive Director/Director of Clinical Operations with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.

 

  • Responsible for compliance with hospice policies and procedures and with local, state, and federal regulatory requirements governing the practice of the assigned team.
  • Actively promotes and directs assigned team regarding quality of care and safety of patients and staff.
  • Engages in thorough problem resolution and complaint investigation.
  • Provides in-services to staff to promote the health and safety of the patient, to familiarize staff with regulatory issues and agency policy and procedures, and to address any identified educational needs.
  • At least 4+ years full-time experience as an RN or equivalent required.

  • Assists Executive Administrator/Executive Director/Director of Clinical Operations with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.
  • Responsible for compliance with hospice policies and procedures as well as compliance with local, state, and federal regulatory requirements governing the practice of the assigned team.
  • Actively promotes and directs assigned team regarding quality of care and safety of patients, as well as staff.
  • Engages in thorough problem resolution and complaint investigation.
  • Provides in-services to staff to promote the health and safety of the patient, to familiarize staff with regulatory issues and agency policy and procedures, and to address any identified educational needs.
  • Assists Executive Administrator/Executive Director/Director of Clinical Operations with employment of qualified personnel including verification of licensure or certification as required by state and/or federal regulation, prior to employment and annually thereafter.
  • Completes evaluations and competency assessments timely and according to hospice agency policy, as well as state and federal regulatory requirements.
  • Assists Executive Administrator/Executive Director/Director of Clinical Operations with adherence to policy and procedure manuals that establish and support the highest possible quality of care to patients and families.
  • Follows hospice agency and company policies and procedures related to human resources expectations and potential disciplinary action(s) for assigned team.
  • Ensures initial and ongoing orientation for assigned team and collaborates with Executive Administrator/Executive Director/Director of Clinical Operations on evaluation of assigned team members.
  • Ensures compliance with regularly scheduled continuing education for assigned team.
  • Assists Executive Administrator/Executive Director/Director of Clinical Operations with oversight of care provided by staff that promotes the health and safety of the patient.
  • Assists Executive Administrator/Executive Director/Director of Clinical Operations to validate that agency policies are enforced.
  • Assists Executive Administrator/Executive Director/Director of Clinical Operations with staff recruitment to ensure that adequate staffing is maintained.
  • Serves as a contributing member for Interdisciplinary Group, Quality Assurance Performance Improvement initiatives, including completion of chart audits and strategic agency planning.
  • Participates in QAPI plan and processes.
  • Serves as liaison between agency personnel and administration.
  • Provides patient care according to plan of care, as ordered by the physician.
  • Maintains strict patient confidentiality.
  • Maintains personnel file, completes assigned education, and completes applicable in-services.
  • All other duties as assigned. 

License Requirements

  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

 

Additional State Specific Requirements

  • LA: a minimum two years full time experience working as an RN. However, two years of full time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.
  • MS: At least one year full-time experience as an RN. However, three (3) years full-time clinical experience in a healthcare setting as an LPN may be substituted for the one year full time experience as an RN.
  • VA: Experience with terminally ill required.
  • AL, AR, AZ, CA, CO, FL, GA, ID, IL, IN, KS, MI, MN, MO, NC, NJ, NV, OH, OK, OR, PA, SC, TN, TX, WA, WV: No additional state specific requirements.
