Brook Street are looking for an administrator for their client based in Fareham town centre.

Become an integral part of our Placement Support Team, dedicated to facilitating swift hospital discharges for individuals under the Discharge to Assess pathway or with standalone health needs.

Work closely with the CHC Commissioning and Central Assessments Teams to streamline referral processes and resolve complex cases effectively.

We're looking for a proactive team player with excellent communication and IT skills, capable of prioritising tasks and working autonomously. Aim to achieve our goal of discharging 80% of referrals within 48 hours, ensuring efficient patient outcomes.

Once training is complete, there is the opportunity to work hybrid with one office day and four remote working days.
