Social Worker - Adult Locality TeamGateshead, UK£19 - £24 per hour

About the Role:

Randstad is recruiting a skilled Social Worker to join the Adult Locality Team in Gateshead. This role involves providing assessment and support to adults within the local community, including older adults and those with physical disabilities or mental health needs. You will work closely with individuals, families, and multi-agency partners to promote independence, improve quality of life, and ensure that individuals receive the appropriate care and support.

Key Responsibilities:

  • Conduct thorough assessments of adults' needs, creating and implementing personalized care and support plans.
  • Work in partnership with individuals, families, and other agencies, including health services, housing, and community organizations, to coordinate and deliver effective support.
  • Advocate for individuals to ensure they have a voice in their care and that their wishes are respected in all decision-making processes.
  • Manage a caseload, ensuring timely reviews, updates to support plans, and compliance with safeguarding and legal requirements.
  • Provide advice, support, and signposting to local resources to help individuals maintain independence in their community.
  • Conduct risk assessments and respond promptly to any concerns, following safeguarding protocols to ensure individuals' safety.

Requirements:

  • Qualified Social Worker with valid Social Work England registration.
  • Previous experience in adult social care, preferably in a locality team or similar community-based setting.
  • Strong understanding of the Care Act, Mental Capacity Act, and safeguarding principles.
  • Excellent assessment, planning, and communication skills, with the ability to work effectively with a range of partners and stakeholders.
  • Commitment to promoting independence, choice, and control for adults in the community.
