The ClientOur client is part of the NHS Trust, working in collaboration with the Integrated Care System, bringing together NHS, social care, public health and other public, voluntary and community sector organisations.

The Special Allocation Scheme (SAS) was introduced as a Directed Enhanced Service, with the aim of providing a secure environment in which patients who have been challenging in their own GP practice can receive general medical services.

The RoleYou will primarily provide clinical assessments for registered patients via both telephone, virtual and face-to-face consultations. These patients could be homeless or be patients that have been removed from their own practice.Your appointments will be 15 minutes each.You will work closely with the wider HHC/SAS team, including a team of mental and physical health nurses, to develop these unique services, building close links and relationships with secondary care and GP service colleaguesYou will need to manage the flow of patients by balancing the clinical need of the patient with the capacity of the provider to deliver the necessary response.You will work in a team, so will not be alone with the patients and consultations will be done from behind a screen, when a face to face consultation is required.

The CandidateYou will need to be self-motivated, have empathy and have an extra level of patience for these challenging patients.You will be a fully qualified GP, registered with the GMC and be on a performers list.You will need to be up to date with all your mandatory training, appraisal,immunisations, and be on the DBS update service.Experience of community-based healthcare would be beneficial.
