We are seeking a dedicated and compassionate Advanced Practitioner for our Leaving Care team. This role is crucial in supporting young people transitioning out of the care system, helping them achieve independence and build a brighter future.

Day-to-day of the role:
  • Conduct thorough assessments of young people’s needs and develop tailored care plans to support their journey to independent living.
  • Provide emotional and practical support, advocating for young people’s rights and ensuring they have access to essential services such as housing, education, and employment.
  • Collaborate with various agencies, including social services, healthcare providers, and educational institutions, to coordinate comprehensive support and resources.
  • Offer guidance and mentorship to help young people develop life skills, build confidence, and make informed decisions about their future.
  • Regularly monitor and review the progress of care plans, making necessary adjustments to ensure ongoing support and positive outcomes.
Required Skills & Qualifications:
  • Diploma or degree in social work, CQSW, CSS, or equivalent.
  • Registered with Social Work England (SWE).
  • UK-based statutory social work experience.
  • Must have passed the Assessed and Supported Year in Employment (ASYE) if qualified after 2012.
  • A full driving licence and willingness to drive.
  • DBS clearance.
  • Experience at Senior Social Worker level.
