Summary:We are seeking a dedicated and experienced Complex Care Coordinator to join our team. In this role, you will be responsible for overseeing the care of our service users and the coordination of our care workers within the local community. Your duties will include monitoring and reviewing care plans, coordinating nurses and care workers, and ensuring that all care visits are allocated and conducted to the high standards expected by our organization. You will manage a wide geographical area and supervise the care provided to service users within that area.

Key Responsibilities:

  • Monitor and review care plans for service users.
  • Coordinate and allocate nurses and care workers.
  • Ensure high standards of care are maintained.
  • Supervise and support care workers and nurses.
  • Enable service users to remain safely in their own homes with as much independence as possible.
  • Maintain regular communication and continuous monitoring to ensure professional, discreet, and caring support.
  • Ensure all service users maintain their right to privacy and dignity.
  • Participate in the on-call rota to provide emergency support 24/7.

Qualifications:

  • Level 2 in Health and Social Care or proven competence in the role and willingness to undertake qualification.
  • Level 3 or above in Health and Social Care.
  • Clinical Qualification.

Experience:

  • Experience working within a fast-paced care service.
  • Proven experience in growing a safe and effective service.

Skills and Attributes:

  • High degree of flexibility due to the nature of the role.
  • Ability to handle high-pressure situations and manage staff absences/issues.
  • Strong communication and organizational skills.
  • Compassionate and professional approach to care.
