About the role:

The successful applicant will be joining a dedicated multidisciplinary team within a specialist mental health service. This role involves working with individuals requiring complex care, supporting them to achieve greater independence and stability.

The Social Worker will manage a diverse caseload, conducting social care and risk assessments, facilitating group activities, and contributing to care planning meetings. Additionally, they will liaise with relevant authorities throughout service users' admissions and discharge processes, ensuring a coordinated approach to care.

Key responsibilities:

  • Educate staff on safeguarding matters.
  • Complete annual social care reassessments within the CPA format.
  • Facilitate access to welfare rights, housing, advocacy, and associated support services.
  • Participate in one-to-one supervision and team meetings.

Requirements:

  • Degree in Social Work.
  • Registration with Social Work England.
  • Understanding of complex mental health needs and risk management.
  • Strong communication and interpersonal skills to liaise with multidisciplinary teams and external agencies.
