As a Registered Case Manager, you will:
- Oversee all aspects of running complex care packages.
- Ensure compliance with the Health and Social Care Act 2008 and associated standards.
- Travel between care packages from Hartlepool to Lincoln three days a week, with two days working from home.
- Attend monthly visits to the Nottingham head office to ensure alignment and collaboration with the wider team.
- Promote a supportive environment and high standards of professional practice.
- Lead by example and cover shift shortfalls when required.
- Participate in local authority provider and engagement forums.
- Liaise with commissioners, safeguarding boards, and specialist professionals.
