Our client is seeking a dedicated Registered Case Manager to join their team and oversee complex care packages across a large geographic area. This role involves traveling between packages of care from Hartlepool to Lincoln, with an expectation of three days of travel per week and two days working from home.

As a Registered Case Manager, you will:

  • Oversee all aspects of running complex care packages.
  • Ensure compliance with the Health and Social Care Act 2008 and associated standards.
  • Travel between care packages from Hartlepool to Lincoln three days a week, with two days working from home.
  • Attend monthly visits to the Nottingham head office to ensure alignment and collaboration with the wider team.
  • Promote a supportive environment and high standards of professional practice.
  • Lead by example and cover shift shortfalls when required.
  • Participate in local authority provider and engagement forums.
  • Liaise with commissioners, safeguarding boards, and specialist professionals.
