About the Role:
Are you passionate about making a difference in the lives of individuals with Learning Disabilities (LD) and Autism? We are looking for a dedicated Referral and Assessment Manager to oversee the referral and assessment process, ensuring smooth transitions for service users into their new homes. As the largest provider in the region, we pride ourselves on delivering high-quality, person-centered care tailored to individual needs.
Key Responsibilities:
Oversee Referrals & Assessments: Ensure the timely and thorough assessment of all incoming referrals for individuals with Learning Disabilities and Autism.
Comprehensive Assessments: Conduct in-depth assessments to determine the needs and suitability of potential service users.
Facilitate Smooth Transitions: Work closely with families, local authorities, and healthcare professionals to ensure a smooth transition into new homes.
Manage Placement & Movement: Oversee the placement and movement of service users, ensuring their new homes meet their specific needs.
Record Keeping & Compliance: Maintain accurate, up-to-date records of assessments, referrals, and placements, ensuring compliance with all regulatory requirements.
Build Strong Relationships: Foster and maintain relationships with commissioners, social workers, and other key stakeholders.
Support Operational Teams: Assist operational teams in providing tailored, person-centered care and support plans.
Contribute to Service Development: Play an active role in service development and continuous improvement initiatives.
What We Are Looking For:
Experience:
Proven experience in referrals, assessments, and placements within Learning Disabilities and Autism services.
Regulatory Knowledge: Strong understanding of regulatory requirements, safeguarding, and best practices.
Assessment & Care Plans: Ability to assess complex needs and develop appropriate, person-centered care plans.
Relationship Building: Excellent communication and relationship-building skills with commissioners, local authorities, and multidisciplinary teams.
Organisation Skills: Highly organised with the ability to manage multiple referrals and placements efficiently.
Problem-Solving: Proactive in problem-solving, particularly around service user transitions.
Travel Flexibility: Willingness to travel across multiple regions as required.
