With a client-centred approach, our client offers exceptional Case Management services and is dedicated to providing exceptional support, empowering people to reach their full potential despite their injuries.

The Role:As a Clinical Associate Case Manager, you will manage a caseload of spinal cord-injured clients, coordinating and assessing their rehabilitation needs and key life transitions. Your role will focus on personalised care, from home adaptations and equipment to psychological support and vocational rehabilitation. You will lead assessments, goal setting, and advocate for the resources your clients need to live a fulfilling life. Your proactive and solution-focused approach will ensure that every client receives the highest level of care and support.

Key responsibilities include:

  • Conducting detailed initial assessments to determine care, rehabilitation, and case management needs.
  • Monitoring progress and revising goals to reflect changes in the client's situation.
  • Working closely with clients and their families to build trust and act as their advocate.
  • Coordinating with a wide range of professionals to ensure the best care outcomes.
  • Ensuring all documentation and case management is accurately maintained.

Requirements:

  • Minimum 2 years’ post-qualification clinical experience. (ideally in trauma, spinal rehabilitation, or brain injury care)
  • Experience working with spinal cord injury clients
  • Strong communication skills, resilience, and a compassionate approach to care.
  • A clinical qualification
  • Full UK driving license and access to your own vehicle.
