About the Role:

As a Community Health Care Support Worker, you will play a vital role in supporting individuals to live independently in their own homes. You will provide compassionate care tailored to each individual's needs, ensuring their well-being, dignity, and independence.

Key Responsibilities:

? Providing personal care, including washing, dressing, and toileting? Supporting individuals with mobility and medication administration? Assisting with meal preparation and household tasks? Encouraging independence and engagement in social activities? Monitoring health conditions and reporting any concerns? Working closely with families and healthcare professionals to provide person-centred care

What We're Looking For:

? Full UK driving licence and access to a vehicle? Experience in community work, domiciliary care, or supported living? NVQ Level 2 or 3 in Health & Social Care (preferred but not essential)? A caring, patient, and professional approach to support work? Ability to work independently and as part of a team
