Outreach Worker - CamdenHours: 37.5 per weekLocation: CamdenRota: Monday to Friday, weekend shift every 5 weeks2-3 times a week 09:00-17:002-3 a week 6:00-14:00Once every 5 weeks late shift 5:30pm-2amAs an Outreach Worker you will:路 Supporting rough sleepers to access appropriate social care services and accommodation options路 Be a first response to referrals, carrying out needs and risk assessments with new rough sleepers 路 Update assessments on rough sleepers already known to the team路 Regularly update the CHAIN database路 Find the best cause of action for each client's scenario 路 Work alongside other agencies including, local authorities, health services, housing providers, the police, and drug & alcohol services
To be successful you will need to:路 Have excellent interpersonal skills路 Be flexible and responsive to meet the needs of rough sleepers路 Have the confidence and ability to manage a caseload of complex clients路 Have the necessary IT skills to collect, collate and interpret data路 Be confident in representing our client at external meetings and with outside agencies路 You must have an enhanced, adult and child DBS check to apply for this post, if you need one, get in touch
