Adult Social Worker - Preparing for Adulthood Team (Greater Manchester) - up-to-£45,718

NonStop Care is collaborating with a forward-thinking local authority in Greater Manchester to recruit an experienced adult social worker for their newly established Preparing for Adulthood Team. This role offers a unique opportunity to support young people with Learning Disabilities, Autism, and neurodiversity, you'll play a key role in supporting individuals with learning disabilities and autism to live independent, fulfilling live

As a Social Worker in the LDA Team, you will:

? Assess the care and support needs of individuals with learning disabilities and/or autism.

? Work closely with individuals, families, and carers to develop personalised support plans that promote independence and well-being.

? Collaborate with health, education, and voluntary sector partners to ensure the best possible outcomes for service users.

? Apply the Care Act 2014, Mental Capacity Act, and Safeguarding principles to support decision-making.

? Prepare court reports and present cases in legal proceedings, particularly relating to the Court of Protection and safeguarding matters.

? Advocate for individuals, ensuring their rights and choices are respected.

? Support young people in transition to adulthood and independent living.

? Manage a caseload effectively, maintaining accurate records and professional reports.

What We're Looking For

  • A recognised degree in Social Care and current registration with Social Work England.
  • At least 2 years of experience in Learning Disabilities and/or Transition Social Work.
