An excellent opportunity has arisen for a Band 6 Occupational Therapist to join a dedicated team working within a community setting.

Setting: Community

Location: Milton Keynes
Position: Full-Time (Part-Time Considered)
Hourly Rate: £25 - £27 per hour
Start Date: ASAP

Key Responsibilities:

  • Conduct comprehensive assessments and provide tailored interventions for a diverse caseload of patients within the community.
  • Develop, implement, and review care plans that aim to enhance patients' independence and improve their quality of life.
  • Work closely with multidisciplinary teams, including social workers, GPs, and district nurses, to provide holistic care.
  • Support patients with rehabilitation and long-term conditions, focusing on maximizing their functional abilities and promoting community integration.
  • Ensure timely documentation, including patient progress reports, assessments, and discharge plans.
  • Provide education and support to patients, their families, and carers, helping them to manage ongoing care needs in the community.

Required Skills & Experience:

  • HCPC registration as an Occupational Therapist.
  • Prior experience working in a community setting or with a strong understanding of community-based care.
  • Ability to work independently while contributing to a multi-disciplinary team.
  • Excellent clinical reasoning, assessment, and communication skills.
  • Experience with a wide range of patient conditions and a commitment to providing person-centred care.
  • Flexibility to work full-time or part-time, depending on individual preferences.
