About Us:Randstad is partnering with the Mental Health Community Team in North Tyneside to recruit a dedicated and experienced Social Worker.In this role, you will provide critical support to individuals experiencing mental health issues, helping them to achieve stability and improve their quality of life through person-centred interventions.

Job Description:As a Mental Health Social Worker in the Community Team, you will work with individuals facing complex mental health challenges. You will conduct assessments, create care plans, and work closely with multi-disciplinary teams to support clients in their recovery and promote their independence.

Responsibilities:

  • Conduct mental health assessments to determine the needs and risks of individuals in the community.
  • Develop and implement care plans aimed at promoting mental well-being, recovery, and social inclusion.
  • Collaborate with multi-disciplinary teams, including healthcare providers, housing services, and other agencies, to provide holistic support.
  • Offer emotional support and practical assistance to individuals, promoting their recovery and integration into the community.
  • Monitor and review care plans regularly to ensure they remain relevant to the client's evolving needs.

Qualifications:

  • Qualified Social Worker registered with Social Work England.
  • Experience working within a mental health or community-based social work setting.
  • Strong understanding of mental health legislation, safeguarding, and relevant policies.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with clients, families, and professionals.
