Seven Resourcing are currently recruiting for a Health & Social Care Officer to start a new role in Nottingham. 

As a Health and Social Care Officer working within Nottingham City Adult Social Care you will be taking referrals for adults across the city including some of the most vulnerable citizens. You will triage, evaluate, and navigate citizens through to the right services depending on their needs.

Responsibilities:

To coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families, and carers and to manage these in an effective, timely and customer focused way.

To manage specialist or escalated calls from Health and Social Care Support Co-ordinators, Health and Social Care Support Advisors or other colleagues.

To make judgements regarding referrals and requests in relation to complex situations, safeguarding concerns and risk management by triaging and coordinating them to make sure that clients are seen by the most appropriate team / service at the right time to meet their needs.

Applicant Requirements:

Experience: Demonstrable and extensive experience in an administrative/business/marketing/customer care or social care environment.

Qualifications: Foundation degree or NVQ3/equivalent

Compliance: Enhanced DBS, Right to Work in the UK

Hours: 37 hours per week, hybrid working
