Social Worker - Hospital Discharge Team

About the Role:

This vital role focuses on providing exceptional support to individuals requiring assistance with complex discharge planning and admission avoidance from the Emergency Department and Frailty Assessment Unit. Working under the principles of the Care Act (2014), your efforts will directly contribute to improved patient flow, reduced hospital stays, and enhanced overall hospital performance. Our model prioritises getting people home as quickly as possible, ensuring they receive the best possible care and support.

Responsibilities:

  • Conduct comprehensive assessments of patient's needs and develop tailored discharge plans.
  • Collaborate with multidisciplinary teams (doctors, nurses, therapists, etc.) to coordinate care.
  • Liaise with family members, carers, and community services to ensure seamless transitions of care.
  • Advocate for patient's rights and ensure access to appropriate resources and support.
  • Maintain accurate and up-to-date records.
  • Contribute to the ongoing improvement of the hospital discharge process.

Requirements:

  • Social Work Degree (or equivalent qualification).
  • Social Work England Registration
  • Experience working within a health care setting (desirable).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and organisational skills.
