Travel: Must be a car driver with access to own vehicle (mileage paid at 40p per mile)

We are looking for a dedicated and compassionate Community Occupational Therapist to join our Dementia Team on a temporary, part-time basis. This is a fantastic opportunity to make a real difference in the lives of individuals living with dementia, providing essential support within a fast-paced and rewarding environment.

Key Responsibilities:

  • Conduct specialist Occupational Therapy assessments and interventions within service users’ homes.
  • Work as part of a multi-disciplinary team (MDT), providing specialist consultative knowledge and support.
  • Manage a caseload of service users, balancing generic and profession-specific responsibilities, including care coordination.
  • Undertake team duty roles, contributing to the smooth operation of the service.
  • Liaise effectively with a range of stakeholders, families, carers, and local community services through written and verbal communication.
  • Collaborate with other agencies to develop and implement individualised care plans.
  • Work autonomously within the multidisciplinary team, with supervision from the Clinical Lead and support from specialist Occupational Therapists.

What We Are Looking For:

  • Previous experience in a community setting, with the ability to work with individuals in their own homes.
  • Mental health experience (previous experience in dementia services is highly desirable).
  • Strong communication and problem-solving skills, with an innovative approach to patient care.
  • Ability to work flexibly, adapting to the dynamic needs of the service and its users.
  • A passion for making a positive impact in the lives of individuals and their families.
