Job Opportunity: Community Occupational TherapistÂ
Location: Telford or Shrewsbury
Contract: Temporary – 12 weeks
Hours: Part-time (22.5 hours per week, flexible – days to be discussed at interview)
Working Hours: 09:00 – 17:00
Start Date: ASAP
Travel: Must be a car driver with access to own vehicle (mileage paid at 40p per mile)
Registration: HCPC Registered
We are looking for a dedicated and compassionate Community Occupational Therapist to join our Dementia Team on a temporary, part-time basis. This is a fantastic opportunity to make a real difference in the lives of individuals living with dementia, providing essential support within a fast-paced and rewarding environment.
Key Responsibilities:
- Conduct specialist Occupational Therapy assessments and interventions within service users’ homes.
- Work as part of a multi-disciplinary team (MDT), providing specialist consultative knowledge and support.
- Manage a caseload of service users, balancing generic and profession-specific responsibilities, including care coordination.
- Undertake team duty roles, contributing to the smooth operation of the service.
- Liaise effectively with a range of stakeholders, families, carers, and local community services through written and verbal communication.
- Collaborate with other agencies to develop and implement individualised care plans.
- Work autonomously within the multidisciplinary team, with supervision from the Clinical Lead and support from specialist Occupational Therapists.
What We Are Looking For:
- Previous experience in a community setting, with the ability to work with individuals in their own homes.
- Mental health experience (previous experience in dementia services is highly desirable).
- Strong communication and problem-solving skills, with an innovative approach to patient care.
- Ability to work flexibly, adapting to the dynamic needs of the service and its users.
- A passion for making a positive impact in the lives of individuals and their families.
