As a Dementia Care Manager, you will:
- Lead and manage the dementia community in the home.
- Deliver dementia-related learning and support to team members and relatives.
- Develop and implement Individual Care Plans, Risk Assessments, and Daily Assignment Sheets.
- Ensure compliance with CQC regulations and Hallmark standards.
- Act as a coach to team members, ensuring resident needs and preferences are met.
- Communicate regularly with families, GPs, and other professionals.
- Lead the delivery of the Dementia strategy at home level.
- Work with the lifestyles lead to ensure a suitable programme of activities.
The Dementia Care Manager role comes with a comprehensive package, including:

- NVQ 4 or 5 / QCF 5 in Health and Social Care or willingness to work towards it.

- Academic qualification in Management and Leadership or equivalent experience.
- Knowledge of adult abuse, safeguarding, and CQC regulations.
- Strong IT, communication, and organisational skills.
