Care Assessment Officer
Key Responsibilities:
- Conduct comprehensive assessments and continuous reviews of care needs.
- Support providers in fulfilling their duties under CQC regulations.
- Identify and implement the least intrusive services to boost independence.
- Proactively address life challenges to maintain quality of life.
- Collaborate with Sheffield City Council, local Primary and Community Health Care networks, and Community Services.
What We’re Looking For:
To be considered for this role, you must have:
- A good understanding of the current legislative framework in reference to the Care Act 2014, including eligibility, MCA, and safeguarding.
- Knowledge of CQC regulations.
- The ability to recommend changes to care plans and understand the responsibilities and tasks associated with procuring services.
- A good basic level of education.
- Customer service NVQ Level 3.
- A valid UK driving licence and your own transport.
- The current right to work in the UK.
- Willingness to participate as a secondary back-up for our out-of-office service on a rota basis.
